All of our referral forms are on this page, as is some other information that may assist Health Professionals in making the decision to send clients to us.
Our services are designed to assist and support customers as they seek better health. Our consulting staff have been specifically selected from our team to undergo extensive, targetted training in these specialist health areas. These staff members work in conjunction with our pharmacists and external health professionals so that a complete and thorough service can be offered.
To refer a patient to one (or more) of our community health programs, please use the referral forms.
For the CAPS Program for continence support, patients must complete the application form with their doctor.
